Campaign 4XX error gives duplicate page URL

I ran the report for my site and had many more 4xx errors than I've had in the past month. I updated my .htaccess to include 301 statements based on Google Webmaster Tools Crawl Errors. Google has been reporting a positive downward trend in my errors, but my SEOmoz campaign has shown a dramatic increase in the 4xx pages.
Here is an example of an 4xx URL page:
http://www.maximphotostudio.net/engagements/266/inniswood_park_engagements/http:%2F%2Fwww.maximphotostudio.net%2Fengagements%2F266%2Finniswood_park_engagements%2F
This is strange because URL:
http://www.maximphotostudio.net/engagements/266/inniswood_park_engagements/ is valid and works great, but then there is a duplicate entry with %2F representing forward slashes and 2 http statements in each link.
What is the reason for this?

I did a crawl of your site and you only have 6 404's i could find
most were pointing to this file
/iepngfix.htcthis file is to fix transperency on png images for IE6
But you do have over 1600 un-necessary redirects,causing a huge lose of link juice.
I asume from looking at a few that you have been 301ing 404's to other opages, you really should find the source of these 404 and remove the links or fix them, this would save you a huge amount of link juice -

Because the links you put are to your private data, SEOmoz won't let anyone follow. Can you put in a screenshot and then just give a link tot the actual url that is a problem? Thanks.

F rated page appearing higher than A rated page
Hi, We have a page that we have optimised in the MOZ system and got a grade A for the term "Dental Implants Wolverhampton" The page that has been optimised isn't being indexed in google http://www.perfectdental.eu.com/treatments-implants.php However, this page http://www.perfectdental.eu.com/uk-clinics-dental-team.php Which hasn't been optimised is indexed 11. If i run this page through the MOZ "on-page checker" it receives a quality score of F. Its very confusing, would be great to hear from anyone with any ideas? Thanks

Have a Campaign, but only states 1 page has been crawled by SEOmoz bots. What needs to be done to have all the pages crawled?
We have a campaign running for a client in SEOmoz and only 1 page has been crawled per SEOmoz' data. There are many pages in the site and a new blog with more and more articles posted each month, yet Moz is not crawling anything, aside from maybe the Home page. The odd thing is, Moz is reporting more data on all the other inner pages though for errors, duplicate content, etc... What should we do so all the pages get crawled by Moz? I don't want to delete and start over as we followed all the steps properly when setting up. Thank you for any tips here.

Too Many On-Page Links
The SeoMoz site crawler says all my pages have too many links. I am using Dreamweaver with a horizontal Spry drop-down menu bar. My site has several hundred pages and about 100 of them show up in this Spry menu bar. I believe that this would be considered a false positive for too many links - am I right? Or is Google seeing this also as too many links per page? I am trying to get my Google rankings back after being hurt badly by the Penguin. I am using php but don't see another way to do the site links without going to a CMS type site. Thanks for any help you can give.

Should I worry about duplicate content errors caused by backslashes?
Frequently we get red-flagged for duplicate content in the MozPro Crawl Diagnostics for URLs with and without a backslash at the end. For example: www.example.com/ gets flagged as being a duplicate of www.example.com I assume that we could rel=canonical this, if needed, but our assumption has been that Google is clever enough to discount this as a genuine crawl error. Can anyone confirm or deny that? Thanks.

How do I get the Page Authority of individual URLs in my exported (CSV) crawl reports?
I need to prioritize fixes somehow. It seems the best way to do this would be to filter my exported crawl report by the Page Authority of each URL with an error/issue. However, Page Authority doesn't seem to be included in the crawl report's CSV file. Am I missing something?

On-Page Keyword Optimization Question
First let me say I want to improve the text of the site I am working on focusing on the site visitor in the first instance. I run the "On-Page Keyword Optimization" The page fails on "Avoid Keyword Stuffing in Document... ...Occurrences of Keyword 48" well over the limit of 15. The occurrence include those in the site navigation and strapline, but it was my understanding that Google was aware of nav areas/areas common to most other pages on the site and that keywords in these areas weren't viewed as being part of the page content. The keyword is the main keyword for the company, and the page is the home page i.e. "acme widgets" the others are "acme widgets for the home"... well you get the idea: The page breaks down as follows: 5 instances in primary nav 1 instance strapline 3 instances secondary nav Remainder in page body I am told by the tool to reduce to 15 instances, so should I? Have 9 instances in the nav and other areas and 6 or so on the page Have 9 instances in the nav and other areas and 15 or so on the page
