Always include the targeted keyword in the alt tag?

Studies show that using the targeted keyword in the alt tag helps to rank better for a keyword. But what if the keyword doesn't exactly describe the image, would you still use it in the alt tag?
For example, say you have an image of the brain, but the article is about cognitive behavioral therapy -- an accurate description would say 'human brain' but an SEO alt tag may say 'cognitive behavioral therapy'--which would you use?

There is some good advice here from. Scott is right that your example is not inappropriate. EGOL is steering you very well also.
When using alt text, most do not take advantage of what it can do for you. You can use around 140 characters and give a good description of the image (as if someone blind was hearing it) along with using keyword relevant text. I recently read that by having your images with good alt text get ranked in images that does have an effect on organic. Now, it wasn't Google saying it, but to a degree it makes sense it would have an effect - how much is anyone's guess.If your example is not close to what you are putting up, go with EGOL's advice and get a new image. (I suggest geo tagging it if at all possible as well).

I usually try to make the first image on the page highly relevant to the primary keyword and use that keyword in the alt. However, if the image was not well described by that keyword I would use something more appropriate.
It might help you with longtail and image search... but I doubt it will hurt much for the primary query... why not get a second image that matches the primary query?

Wow, that's a really good question. If there were only one image on a page (e.g. selling a product), I'm sure that labeling it as you suggest wouldn't get flagged as keyword stuffing. Certainly you want to enrich the human experience, but to be honest, I haven't noticed as much impact with the alt tag as I have with other methods. I'm interested to see how this discussion evolves.

I would use alt tags to accurately describe the image. Google should be able to tell that an image of a human brain is closely related to cognitive behavioral therapy and will help add to the content and quality of the page. It might even look spammy to have "keyword stuffed" alt tags.

Keyword research
Do I understand it correctly ? Let's say my keyword is Piedmont bike tours. From what I understand I need to use synonyms of Piedmont bike tours that people would use to find my website such as bike trip in Piedmont or cycling vacation in Piedmont and then closely related phrases in my content which are not necessarily synonyms but that semantically related, correct ? Do I need to do that for every topic that I decide to talk about on my webpage. For example let's say another topic I have decided to talk about on my page about Piedmont bike tour is Barolo. Do I need to do the same process for Barolo ? Thank you,

Are ALL duplicate title tags bad??
We’ve had some success recently by reducing the number of duplicate title tags on our website. We have managed to fix all the simple cases but there are a number of stubborn examples that we don’t know how to fix. A lot of the duplicate tags come from the website’s forums. Many questions have been asked multiple times over the years where the user has phrased the question in the same way. This has led to many cases where different forums posts have the same title tag. For example, there are six title tags with the words ‘’need help”! These are being highlighted as duplicates and currently we have several thousand of these. Would this be a problem? I’d be tempted to say that we should leave them as they don’t seem unnatural to me. One solution other solution we are considering is to append the forum name to the question to any post after the original, falling back to appending the date if that doesn’t distinguish it. Do people think that this is a good solution to implement or would it be better to leave these duplicate title tags as they are? Any help would be appreciated 🙂

Keyword Selection - Long Tail or not long tail keyword
Hi Friends, We have a keyword "plus size clothing" but we used this as "plus size clothing 2012" to Cover the both keywords "Plus size clothing" and "Plus size clothing 2012" My question is, are we still focusing on "plus size clothing" when use "plus size clothing 2012" instead of the main keyword.? What strategy do yo suggest that when to use long tail and when to not. (when I talk to use a long tail than it means use it prominently on the page as main keyword should be used.)

Lost Google rankings for the main keywords
Hello everybody, I love seomoz and was enjoying all the benefits it has, was reading Q&A and implementing new marketing strategies for my website. I would really appreciate if anyone could explain my situation. The website, to which I devoted last 12 months of my time, was suddenly dropped from it's main keywords to nowhere 2 days ago, it does not look like a "dance" because some of my competitors lost their rankings too. I think its a penalty. The site http://goo.gl/3VTMq is almost 2 years old, of the highest quality content. I've been adding content slowly over 24 months, building backlinks slowly, the rankings also were going up slowly. Here are some of the keywords I've been ranking for: car insurance companies - 12, best auto insurance companies - 1, top car insurance companies 1, cheap auto insurance - 16, low cost auto insurance 4,lowest auto insurance - 2, and many more... According to seomoz, I have no major problems with on-site seo, I have low bounce rate, high pages/visit and high time on site. There is only one left - incoming backlinks. Google says that it just can't hurt you, but I think this is the only explanation of my problem. My backlinks consist of everything - from high quality contextual backlinks, to lower quality comment backlinks. I used Press Release services too.. This problem is like a nail in my head, I look forward to hearing your response. Thanks in advance, Vaz

Keyword Self-Cannibalization Concern
Right now I have an E-Commerce website that has layered menu properties. I have one page trying to rank for "NextGen Digital Ballast" that is the main category page. However, on that category page I link out to three product pages which would be "NextGen 400W Digital Ballast", "NextGen 600W Digital Ballast" and "NextGen 1000W Digital Ballast". The on page ranking factors tools is saying I may need to consider making adjustments because of the potential self-cannibalization, but I wanted to get some feedback to see what others thought about that.
