Why has my site gone from 3rd to 21st in Google...HELP

Okay I stared my own web development business about 8 - 9 months ago and decided that I was going to target a small number of local keywords that were relevant to my business and geographical area.
So I decided to target the keywords below and and started to get good traffic, I never have bought links and have not copied any content although I know i should try and add more content to my website.

However recently my website has dropped massively in the SERP's and wanted to know what has caused this (i know Penguin) and what I can do to improve. I have listed below my rankings to show you the drop:

I decided to change my homepage Page Title a week ago to make sound less spammy but this has made no difference and wanted some help on what has happened so i do not do this again and what I can do to improve.

Thank you for getting back to me all help is great.
The other site in question (www.nothampton-websitedesign.co.uk) is owned by me, I have created but have not done anything with the to date. My plan was once I was ranking well for the Milton Keynes site then I would start on this site, adding all new content etc.
I think I will remove all the content from the Northampton website until I am ready to work on this.
With regards to SEO chat thanks, I was not aware of all the comments. I read one comment which was not very nice so decided not to go back. I shall now read all the posts.

Probably a good idea to read both of the blog posts below and follow your own advice on duplicate content. (these are just two examples). You might be cutting your own throat....

You have not used enough anchor text keyword variation. You were hit by Penguin. I ran a report on Open SIte Explorer (seoMOZ tool) and it seems like you're over optimizing your target keywords.
You can go for these keywords but you should not be so obvious. The goal is to help the user without the search engine. However, its always good to know the search engine law.

On Site Problem Caused Rankings To Drop?
I'm getting to the bottom of why my site dropped rankings on the 9th March. I noticed that in google there is a cached version of my site on 9th March at 8.37am which is when my rankings dissapeared. Presumably this is when google last crawled my site? I guess this means that google found something on the home page or on the site that it didn't like? I wonder if anyone can take a look and let me know if there's anything obvious. Could it be a duplicate content penalty as I have lots of categories pulling content from the same posts?

Mentioning own site and keywords on here?
I have noticed that sometimes posters will talk about a site without mentioning what it is. I assume this is because it one of their clients so there is confidentiality, is there any other reason I should be aware of? its just that as I am new I am usually cautious and am considering posting my own site and mentioning all my keywords to ask for people’s verdict for my on-page SEO. two part question on original content How would you go about checking if a site holds original content accept the long search quary within Google? ans also if I find many sites carrying my content and I am the original source should I replace the content? thanks

Help with Appropriate Use of Rel Canonical
Whenever i enable Canonical URL through the 3DCart Control panel I get this Critical Factor error when running the on page report card: Appropriate Use of Rel Canonical Moderate fix <dl> <dt>Canonical URL</dt> <dd>"http://rcnitroshop.com/Nitro-Monster-Truck"</dd> <dt>Explanation</dt> <dd>If the canonical tag is pointing to a different URL, engines will not count this page as the reference resource and thus, it won't have an opportunity to rank. Site structure for services and blog articles
Hi, looking for some advice on the structure for a relatively small site (around 200 pages). I'd like a structure where we can talk about our services as well as write blog articles on topics that relate to our services. We'll have loads more content in the blog area than in the services area. I was thinking of this: option 1: /services /services/copywriting
