Canadian Local SEO gurus: What are the best citation sources?

I have been trying to gain some position in our local O-pack results and have used whitespark to hunt for citations and aquire as many as possible, but even after 4-5 months google has still not detected many of the citations we have added. What are you favourite citation sources? which ones really help? i.e. what are some quality canadian citation sources?

Either you've never had to deal with local search optimization, or you are just answering questions with fake answers for the moz points.

Citation is not a popular word in the US seo community. Is there another word that can describe what you are talking about?

Is city name really a no-no in Google+ Local description field?
I know that adding one's city name in the business name and category tags for Google+ Local is a no-no. And Google says so in its quality guidelines. But what about in the business description? I don't see that in Google's guidelines. Can anyone clarify?

Local Ranking Factors Question
Local SERPS continue to confuse me. I feel like I can tell what's going on but I'm never right. How do these two listings rank for "best bar" in the local Chicago SERPS? I've analyzed everything and there aren't any good indicators as to how this was done. The former doesn't even have a website. The latter example has some category keyword stuffing, which I don't get why they're account hasn't been suspended because of it. It also has a very small and not very powerful link profile. Any insights would be much appreciated!

SEO & Mass-Renaming of Images: Image Name, Alt Text, etc.
We are currently in the process of changing all of our image names and their alt-tags to be descriptive of their content. For instance, changing image names from image0001.jpg to blue-cell-phone-cover. We have hundreds of images on our site, would it be best to rename all of the images and update their tags in one sweeping change or trickle the changes, uploading changed images every other day or so?
