Why did my home page fall off of google rankings?

My home page at www.smt-associates.com has been ranked well for various key word phrases for years. I've tried to optimize it for the search "Crystal Lake CPA Firm" and it always had ranked number 1-2. Now it doesn't even rank in the top 5 pages (actually I don't know which page it falls on). I did an on-page report card and it has an A rating. So, what is preventing Google from ranking my home page on page 1? There's not that much competition so this should be an easy ranking for me.
I don't know how ling this has not been listed, but I did modify my site about 12-18 months ago with a new WP theme. Could the theme be the problem?

Yes, they will most often combine now. If you click the main link, it will still go to your website, with a link to the google+ page underneath. If you have a second page that ranks for the same keyword, it will show up as a normal listing.

Is the Google+ page listing supposed to be combined now? I thought I used to be in both spots.

I just did a search, and you came up #1 for "Crystal Lake CPA Firm." It was merged with your Google+ page. I'm not sure what you're seeing. Did you look at the local listings too?

Hi, I'm having a strange problem from past one month. My website gets about 10k pageviews a month with an average bounce rate of 50%. Lately, I observed a strange thing. The avg time on page of visitors from selected countries is less than 1 second and the bounce rate is zero!! How is this possible? What does "on first page" mean in seomoz ranking reports?
Hi - When reports here show numbers of keywords appearing "on first page", there must be some implicit assumption made about the number of results listed per page. 1. Can anyone tell me what that assumption is? Is it 10? 20? 2. What about universal results Local links? If the answer to number one is, for instance, 20 results per page, then are there any assumptions made about the number of universal results Local links included? I'm just trying to understand what the reports mean. Thanks, Tim

Too Many On-Page Links
I'm getting the warning of "Too many on-page links". I have a number of affiliate marketing sites, all Wordpress, all with sidebars. In the header navigation bar is a link that offers "reviews" and links to each product/brand. This is not a drop down link but leads to a page with product links. Also, in the sidebar I have links that also lead to the products/brands. Redundant yes, but in the beginning this seemed to be a good practice in site design. It would be easy to simply remove the widget that contains the link process in the sidebar. The problem may simply be that the sidebar is the same for every page in the site(s). Is this hurting total SEO as my sites are all over 95-percent indexed. One site in particular does very well in traffic and sales so is removing these links potentially going to improve my SEO and ultimately by success? Thanks, Don

Duplicate page content
I have a website which "houses" five different and completely separate departments, so the content is separated by subfolders. e.g. domain.com/department1 domain.com/department2 etc. and each have their own individual top navigation menus. There is an "About Us" section for each department which has about 6 subpages (Work for us, What we do, Awards etc.) but the problem is that the content for each department is exactly the same. The only difference is the navigation menu and the breadcrumbs. This isn't ideal as a change to one page means having to make the change to all 5 and from an SEO perspective it's duplicate content x5 (apart from the Nav). One solution I can see is to have the "About Us" section moved to the root level (domain.com/about-us) and have a generic nav, possibly with the department names on it. The only problem with this is that it disrupts the user journey if they are forced away from the department that they're chosen. Basically i'm looking for suggestions or examples of other sites that have got around this problem, I need inspiration! Any help would be greatly appreciated.
