On-page Optimization - Is an A grade absolutely necessary?

It is commonly stated that three exact keyword mentions on a page is best practice for SEO. Recently, however, I have noticed a few articles that have hinted against it. matt @highonseo briefly mentioned the subject here: http://www.seomoz.org/blog/going-beyond-moz-metrics-to-answer-why-is-this-site-outranking-me
Also, one our clients competitors is ranking above them with only one exact mention of the keyword in the page title with no exact mentions in page copy. I know other factors can make a difference, but this is does raise questions of our on-site strategy.
I haven’t found any literature that specifically recommends using one exact match of a keyword, so any opinion would be very useful!

I think that the page titles, URLS, and H1 tags are best for exact keyword placements. Using them in the page copy can often look out of place.
For example, your keyword might be “Car Repair London”. Inserting this as an exact form into the page copy could read something like “With 30 years of experience, our Car Repair London”.
Google is looking for user experience at the end of the day. What are your thoughts on making the page look as natural as possible?

It is still an SEO best practice to use the exact match keyword on your page.
What Matt is talking about in the article is keyword stuffing, which is overusing a keyword on a page in an unnatural way, which can get you penalized. Including 2-3 mentions of your keyword on your page will help not hurt you and can help increase the relevance of the page in Google's eyes.

I recommend targeting themes of keywords rather than one exact match keyword. I typically choose the top 2-3 keywords per theme to target based on highest search volume and lowest competition.

Grade F page on Moz positions No 1 on Google Keywords not contained
Hi I am trying to understand why a page list in position 1 on Google despite the fact it does not include the search terms anywhere in the page source. One of our sites has been in that position for years has great content and links for the key word terms so how can the other page overtake it and all of the other keywords without so much as a sniff of the keyword in the URL, Meta, content or images. It grades F on Moz! How can I discover the technique that has been used. This really is black art stuff or do Google accept payment from major corporations to list their pages irrespective of content?

Home Page Keywords not Ranking and Assigned to Inside Pages
Hi, thank you for taking the time to read this. We have a few websites with the same problem. I will use http://www.prepared-meals.com as an example: The home page was ranking on page one for keyword "Prepared Meals". The site is about 6 months old. We use the Moz page optimizer on all pages of our websites to score an A rating. Recently we found the home page is no longer showing up in search results and the keyword "prepared meals" now points to an inside page that is not relevant: http://www.prepared-meals.com/Senior-Meals/Moms-Meals-Reviews.html this page shows up for Prepared Meals around page 15 in Google results. We have read keywords in the URL might be the issue, even though the page optimizer in MOZ says to do that. We are wondering if this is the issue or there is some other problem we are not aware of. How should i optimize this page
Hi, i am having major problems in optimizing this page as it is a magazine site. Jobs listing page optimization
I've a client with a jobs page. I've found that a high traffic, low competition keyword phrase 'OH jobs' and wish to optimize appropriately. The trouble is that there is hardly any body text and any number of job entry listing. As the listing which all go to individual job description pages, naturally fit the description (part description) OH Job - would it be good to use this description within the anchor text of the links? If so is there a maximum? I've been trawling around SEOmoz and Google Search and have got more and more bogged down! ta

The need of two-keyword optimization in the same page
Hi there! Due to the business model of my company I have to optimize two keywords in one page. I just asked about this question before and someone told me thant as long as they refer to the same concept and have almost the same "meaning", it is possible. The problem is how to face it up. hi there! Is it possible not to be in the first 20 or 30 positions in the SERPs after executing onpage SEO actions (keyword optimization, metatags, ....) even for keywords for which there's not "too much" competition? Is there a way of visualize the pages indexed by the google bot? (the pages especifically, not the number) in order to discard indexing problems? Thank you!

Pages vs Posts
What are your thoughts on pages vs posts? I am setting up a new blog for a client but not sure how to structure the content. I may just do posts or a whole bunch of page listed down the sidebar. It seems like my pages always rank better than my posts. Has anyone else noticed this? Could it be because of the dates tied posts?
