Why are Google Webmaster Tools' Google rankings different to actual Google rankings?

Dear Moz,
We have noticed that according to Google Webmaster Tools one of our client sites is ranking very prominently for some of the major key phrases that we are trying to rank them for.
However, when we perform a Google search for these queries, our client's content is nowhere to be seen, not even on the 5th page (we logged out of the Google account before performing the test).
A long-term manual spam action on our client's site was recently lifted by Google - is it possible that Google Webmaster Tools is providing data about our client's estimated Google rankings, without taking into consideration the penalty of the manual spam action which was taken?

I'm not sure Google Webmaster Tools takes into consideration a person's IP address when they list Google rankings for a website in Google Webmaster Tools. These rankings are linked to Google Analytics for a client website.

Do a search on Google search personalization. Example

What you see when you search is based on if you are logged in (or not), your IP, your previous searches etc. These days there is no single ranking level, you are seeing a sort of average. Rankings are more useful as a relative measure vs an absolute. Are you moving up or down, is that causing increased or decreased traffic and is that traffic converting at a higher or lower percentage level. You really need to look at all of that together and use ranking as a way to diagnose any issues with your final conversion numbers.

Google Search Console Not Indexing Pages
Hi there! I have a problem that I was hoping someone could help me with. On google search console, my website does not seem to be indexed well. In fact, even after rectifying problems that Moz's on-demand crawl has pointed out, it still does not become "valid". There are some of the excluded pages that Google has pointed out. I have rectified some of the issues but it doesn't seem to be helping. However, when I submitted the sitemap, it says that the URLs were discoverable, hence I am not sure why they can be discovered but are not deemed "valid". Our sitemap is not indexed i Google even though it's successfully processed
Hi, Ours is a WP hosted website. We have submitted the XML sitemap with a WP plugin. It's been successfully processed by Google but it's not been indexed in and can't be found in SERP. How to get this indexed? Will there be any low crawling of sitemap as it's not indexed? Thanks

Best approach to ranking locally
Hello, What are the best approaches to ranking locally. Ie a user in Dallas googles "mechanics" and local results return. I understand Google+ local pages to be an important factor for the location based listings on maps. What about about location specific pages on the site? Meaning if we have a page on the site talking about areas we serve in Dallas. Other suggestions?

Why has my homepage been replaced in Google by my Facebook page?
Hi. I was wondering if others have had this happen to them. Lately, I've noticed that on a couple of my sites the homepage no longer appears in the Google SERP. Instead, a Facebook page I've created appears in the position the homepage used to get. My subpages still get listed in Google--just not the homepage. Obviously, I'd prefer that both the homepage and Facebook page appear. Any thoughts on what's going on? Thanks for your help!

Can visitors duration time affect Google Rankings?
Does the time a person stays on a website affect Search Rankings? If so, could the lower time from Adwords Visitors be effecting organic rankings? And the same for bounce rate. If Non-Paid Search Traffic Avg. Visit Duration time is 3:55 and Paid Search Traffic Avg. Visit Duration is 1:59 Could the low duration time be affecting our website rankings?

SEO updates and rank changes
We have been updating page titles and meta descriptions for a client (not changing ANY links and the content we are replacing is "fluff," no major keywords or any relevant information) yet in the past few weeks, rankings have plummeted. I used the SEOMoz grader to check and make sure we have the keywords in there, in the right places for the updated page source info, and we're getting A's yet for those same keywords, the website is nowhere to be found. For example for the phrase "organic t shirts," we get an A for this page: http://greenpromotionalitems.com/organic-t-shirts.htm but when searching organic t shirts, no Green Promotional Items... Ideas?

Difference in which pages Google is ranking?
Over the past two weeks I've noticed that Google has decided to change which pages on our site rank for specific keywords. The thing is, this is for keywords that the homepage was already ranking for. Due to our workload, we've made no changes to the site, and I'm not tracking any additional backlinks. Certainly there are no new deep links to these pages. In SEOmoz dashboard (and via tools/manual checking with a proxy) of the 24 terms we have first page ranking for, 9 of them are marked "new to top 50". These are terms we were already ranking for. Google just appears to have switched out the homepage for other pages. I've noticed this across a couple of client sites, too, though none to the extent that I'm seeing on our own. Certainly this isn't a bad thing, as the deeper pages ranking means that they're landing on the content they want first, and I can work to up the conversion rates. It's just caught me by surprise. Anyone else noticing similar changes?
