Logistics of getting links removed

I recently got an unnatural links penalty notice from Google. Well, at least it's the type where they say they're only penalizing the links, and not the site.
I've noticed lots of links from low-quality sites in my Webmaster Tools, and have been wondering why my site hasn't been ranking better since it abruptly dropped off around October 22, 2012 (I know, that's a very long time ago – I've been neglecting the site for other parts of my business).
I'm hoping that if I make an earnest attempt to get these links removed, disavow the others, and submit a reconsideration request, my rankings may improve. So my question is this:
How, logistically, do you go about getting all of these links removed?
I mean, technically I get it: go through all of the links, try to contact all of those webmasters, keep records of all correspondence, and share that with Google. But how? Are there special tools and services for this? Are there contractors you recommend? I have other things I'd rather worry about in my business than undoing these unauthorized links.

If you have only a couple of hundred backlinks to analyze, you're probably better off doing the analysis yourself "by hand". If, on the other hand, you have many hundreds/thousands of links, you may use an automated tool such as: http://www.linkdetox.com/tour/.
I haven't used the tool myself, but seems to cover most of the operations necessary to get rid of links.

The majority of incoming links to a prospect's website are from website pages apparently created solely to provide links to the website. Few have high spam scores. The sites linking to the main site have versions of blogs with linked text. They seem to be providing positive SEO value now, but I'm concerned they might get noticed and hurt the main site in the future.

What is a good ratio of total links to linking root domains?
Is 100 total links for every linking domain too high? I suppose I could also look at ratios of sites that are doing well in the rankings.

Link in the footer

Will removing old (3 years+), low quality inbound links potentially improve my rankings?
I have a client site with thousands of old very low quality links purchased from directories etc over several years - I am certain these are doing nothing for the current rankings and I advised them to stop buying these links 18 months ago when I first started working with them, but do you think pro-actively getting old poor quality backlinks removed will potentially improve the site's rankings?
