Over-Optimized Website

I'm looking for advice for what you would start with if you were working on a website that was extremely over-optimized for 1 keyword. So, for example, I'm going to pretend this client is a dog trainer in Toronto (I can't publicly post the URL).
I've read places that having exact-match anchor text links to inner pages in the footer of the site can cause problems and removing it has resulted in big ranking jumps. I'm looking to see if there are other big items that you would tackle first if this was your client.
Some examples of things the site has:

There is a page for dog training under their Services menu. However, internal links on their site link "dog training" to both the homepage and to this service page. Is that going to cause issues?

The anchor text for internal linking is almost always the exact same word - "Dog Training".

There is a banner that goes across the top of the site that appears on every page that says "Dog Training Toronto". I'm guessing I should remove that. Would the same keyword being overly used on every page cause confusion?

I'm looking to see if anyone has general tips on where to start with a site that has been over-optimized for 1 keyword. He actually has a ton of good content on his blog that gets a ton of traffic (because it's actually useful) so it's not that his content sucks - it's just been overly structured and SEO'd to death.
I found a few articles on this but other than the footer advice I didn't find too many case studies of others that have run into this issue and done a few steps that actually worked.

I would start with keyword research and come up with a large bank of alternatives for the "dog training" keyword. Look for synonyms, alternate word endings (train, trained, trainer), and co-occurrence terms. Then rewrite the content with these to be far more natural. It may require some heavy rewriting, depending on how bad it is.
Definitely get rid of the over-optimized footer links. If you do all of this, you shouldn't have to worry about the banner in the header. Header content is often the same across an entire site.
The hard part may be explaining to your client that on-page optimization isn't so much about exact-match keywords as it is about topical authority.

Best SEO extension or plugin for on-page optimization
I was wondering what you guys might recommend for a browser extension or plugin that would help developers optimize each page for specific keywords. I'm looking for a tool that will give me a grade on how well it's optimized with a recommendation checklist. I would use SEOMoz's onpage analysis tool but it doesn't have access to my private development environment (hence the need for a plugin.) Recommendations? Thanks!

Optimize Page Title - Advice
I am currently going through my site and re-doing the page titles to try and optimize each specific product's page. I know that having the part number as the first piece of text in the title is the best practice. My question is, if i add a bunch of terms to the title, after the part number, is that taking away from the important part (the product number)? For example, my product is Audi A4. The term i want to optimize for is Audi A4. Which would be the better title for ranking purposes? A) Audi A4 | Automobile or B) Audi A4 | Automobile | 4 Cylinder | Made in Germany Thanks for the advice!

SERP listing of a websites' 'categories'
Hi all, just wondering if anyone has thoughts on what I can do to encourage SERP listings that include website categories, eg http://www.google.com.au/search?q=seomoz&ie=utf-8&oe=utf-8&aq=t&rls=org.mozilla:en-US:official&client=firefox-a . I'm assuming search engines only display type of listings when the search query closely matches the domain name? Thank heaps!

I need quality backlinks. Do they need to be similar to company website?
Our website seems to be well optimized. However I need to find more quality backlinks. I am having a hard time with this. Do backlinks have to related to the company? The reason I ask is because I can receive some links, but they do not relate to our company. BUT sometimes we have customers come to our facility and we book them a local hotel room and send them to near by restaurants. Is it ok to have local restaurants and local hotels links on our website and have them link to ours, on a page for customer visits. OR any other tips on receiving quality backlinks?

One domain with keyword optimized pages or multiple domains
Hi There. I have a client in the real estate law services business. Which is better for long term search traffic? 1.
