Keyword stuffing - on page grader count

I used the on page grader for my homepage, www.cprnj.com, and it said the keyword "physical therapy" was used 26 times in the body of the page. But I can't find more than 4.
How exactly is keyword stuffing accounted for, and is there some place that I am not seeing the keywords?

Hi Jason. While it is a best practice to use the keyword in the URL, I think you did overdo it and are running into something that would be considered keyword cannibalization within your own site. While an old blog post, the core principles remain the same here: https://moz.com/blog/how-to-solve-keyword-cannibalization. To get around using 'physical therapy' excessively, you can break down the different types of PT into: treatments, exercises, stretches, and so on. This semantic use of similarly related keywords is a positive signal versus the negative of keyword stuffing. Cheers!

- with regards to the links, from what I've read, a lot of SEO suggestions suggest that you place your most important keyword in your url links, ours being "physical therapy". Did I overdo it a little? I thought it was very basic and descriptive overall, i.e. we do vestibular physical therapy, so the url is /vestibular-physical-therapy
- the moz on page grader says the term was in the body 26 times. I counted 4, and it looks like you counted 4 as well. is there somewhere else I'm not looking?

Thanks for the help! I've been trying to figure out the H1 tag stuff, but I'm using Squarespace and am trying to figure out how to get access to that portion.
Also, with the title tag, unfortunately, I'm not sure how I can shorten the homepage title tag. I feel like I've shortened it as much as possible, basically, most important keyword (physical therapy), location (jersey city, elizabeth), and company name. I've been debating on removing the company name, but it's the first thing that shows up on our Google search result listing. Thoughts?

Ryan is right from a technical view.
But, i would be very hesitant to change the page based on the onpagegrader. My assumption is that google looks at a website and takes the content a customer can see. The fact that the keyword appears in the links seems somewhat high, but you are a into physical therapy! ( the alt text seem varied already)
So unles you have a problem with google i would not focus on that.
Focus on the fact that you should have just 1 H1 Tag ( prefereably not a picture) and the way to long title tag. Fix the / non dash duplicate content problems etc etc..

Hi Jason. Beyond the body text, the term physical therapy shows up heavily in many ways.
- Alt text. 6 times
- Tag Anchor text. 12 times
- As 'physical-therapy' in links. 260 times.
- As 'physical+therapy' in links. 21 times
- Body text. 4 times.
If you look at the code, it's a pretty heavy usage of the term on the home page and throughout the site in on way or another. I'd work on reducing the presence in links and making alt text more descriptive of the image being described than as a box to tick for the keyword location. Cheers!

Can I export data from Keyword Difficulty on several keywords?
I know that you can export data from the tool when only analyzing one keyword. But can you also export Keyword Difficulty Score when there are multiple keywords? On Page Grading Not Working
I am not getting any results in on page grading. When I click on add keywords a list of keywords appears that I have previously entered. They have tick boxes next to them and you can filter them but SEO moz is not automtically creating grade reports? Any ideas? Obviously I've set it up wrong somehow.

Is it possible to exclude pages from Crawl Diagnostic?
I like the crawl diagnostic but it shows many errors due to a forum that I have. I don't care about the SEO value of this forum and would like to exclude any pages in the /forum/ directory. Is it possible to add exclusions to the crawl diagnostic tool?

Too Many On-Page Links: Crawl Diag vs On-Page
I've got a site I'm optimizing that has thousands of 'too many links on-page' warnings from the SeoMoz crawl diagnostic. I've been in there and realized that there are indeed, the rent is too damned high, and it's due to a header/left/footer category menu that's repeating itself. So I changed these links to NoFollow, cutting my total links by about 50 per page. I was too impatient to wait for a new crawl, so I used the On Page Reports to see if anything would come up on the Internal Link Count/External Link Count factors, and nothing did. However, the crawl (eventually) came back with the same warning. I looked at the link Count in the crawl details, and realized that it's basically counting every single '<a href'="" on="" the="" page.="" because="" of="" this,="" i="" guess="" my="" questions="" are="" twofold:<="" p=""></a> <a href'="" on="" the="" page.="" because="" of="" this,="" i="" guess="" my="" questions="" are="" twofold:<="" p="">1. Is no-follow a valid strategy to reduce link count for a page? (Obviously not for SeoMoz crawler, but for Google)</a> <a href'="" on="" the="" page.="" because="" of="" this,="" i="" guess="" my="" questions="" are="" twofold:<="" p="">2. What metric does the On-Page Report use to determine if there are too many Internal/External links? On-Page URL
Hopefully I am missing something basic... I can't see how to specifically add and delete On-Page reports. It seems like running a report adds it but how to delete? Also, how does one change the URL for a report? I have re-organized some pages and can't seem the get the on-page report to keep my URL change. Here is what I tried. From the On-Page report card for a keyword I changed the URL and ran the test. Test runs ok but if I navigate back to the summary my old bad URL is still there.

I would like to export the keywords from all of my campaigns but I can't find anywhere to do that. I don't need data, just the keywords. I want to do an audit and refresh of what I am tracking and excel would be quickest. Thanks!

Rankings not surfacing all my keywords
Usually they have a little day icon of when they will be updated, but I did some adjusting from around 65 to 98 keywords, but only 54 show up. Is this normal? Do I just need to wait? Some of my page 1 keywords are MIA.

HTTP 404 for 404-page?
Hi Mozzers! SEOmoz just finished crawling one of my websites and this crawl found 3 errors. One of these errors was the (custom) 404-page, because of the http-status 404. What's you suggestion about this? Should a 404-page have a status 404? Thanks in advance for your suggestions!
