Cannot re-activate PPC after GWT violation has been removed.

Google adwords not allowing us to reactivate after GWT violation removed and site deemed clean by Google.
Attempts at reaching them for guidance have been unsuccessful. Any suggestions?

I've been through something like this once, and, unfortunately, even with substantive ad spend, it took a lot of time and effort to get an AdWords rep involved. You have a couple of choices:
(1) Complain very loudly, including Twitter and Google Webmaster Forums. Yes, it's not the ideal solution, but it works more often than it should.
(2) Contact anyone you can at AdWords and try to get rep time. I'll tell you what we did - I'm not saying you should do it, but desperate times, etc. We went to a Google Analytics meet-up and got a few cards from Googlers. Then, we emailed all of them. Since they were out of town (for the meet-up), we got their vacation auto-responders, so we emailed all of their bosses. This continued until some VP got so annoyed he demanded someone assign us a rep to shut us up.
Look, I'm not proud of it, but that client got their ban lifted.

we ended up getting a violation because our website was infected with maleware. We successfully identified the code, removed it and subsequently were noticed by Google that the site was now clean.
During the period when the site was infected, our adwords campaigns were suspended by Google, which made total sense. We never got an specific notifications from the Adwords group stating they were suspending the campaigns.
We assumed that once the site was determined to be 'cleaned' by Google, our Adwords campaigns could be re-activated.
So far, our efforts to reach out to Google have failed. No communications and no understanding what still remains to be addressed.
Any suggestions or contact information would be appreciated.

Hi,
Site suspended due to GWT violation? I presume you must have received email from Adwords team about reason of suspension. I have never heard that I have gone through many site suspension reason but it is first time I am hearing this that site penalized due to GWT violation.
can you tell me what kinds of GWT violation occurred ?
You can also contact them (adwords support team ) to resolve the issue suggested by Andy.

Correlating Form Submissions to PPC vs Organic - COA
I currently don't use a landing page model. From either Adwords or Organic you reach our site, find the event product that appeals to you, and then fill out the form. I need a way to determine which of those form submissions came from Organic Vs Paid so I can calculate my cost of customer acquisition (COA). Via Google Analytics I can see that X amount of Organic were submitted and X of Paid. If I get 5 submissions in one day there isn't an efficient way to correlate the form submissions to the medium. Any suggestions on an attribute method that would help me sort out COA?

PPC Adwords Trademark Protection
Hi Wheres the form for filling in/registering your brand with Google for trademark protection in adwords ? I can only find an infringement/complaint form, which given no one currently infringing i imagine will be pointless filling it in. Can I dynamically add city name to my PPC ad text and URL based on the user's search?
I have looked into DKI (Dynamic Keyword Insertion), but have not found a solution and thought that some excellent Mozzer might be able to help. Here is the idea: We have landing pages for hundreds of cities. The local content on each of these cities changes page to page, however the keywords that we are going after are the same. So, I am trying to create a dynamic ad group that looks something like this: Headline: {City Name} {Keyword} Description: We cover {City Name} {Keyword}, get more info now! Will a javascript rewrite of city names on a home page from PPC traffic affect SEO negatively?
We have a client that was originally based out of Orlando and the entire website is SEO'ed for Orlando. In the long term we will start SEO'ing for Tampa. But In the short term, we want to drive traffic from Tampa to the website but we want that traffic to know the business specializes in Tampa service (he has local teams there). We are already using a ?_vsrefdom=changemynumber to change the phone number dynamically to a tampa area code for tracking purposes. We want to dynamically change all iterations of 'Orlando' with 'Tampa' using the same javascript. Does this cause any SEO problems or trip any alarms with Google?
