Difference keyword and co-occurence

Could someone explain me what the difference between a keyword and a co-occurence is ?

"keyword " is the exact string of letters identified as a search term for which someone may want to rank.
"Co-occurrence" relates to terms that are similar to the keyword or words that often show up near, or along with, a particular keyword.
The "Co-occurrence" thing kinda showed up originally as a foil against early article spinner-type spam tactics that just threw a bunch of words together into sentences and paragraphs peppered with keywords without accounting for actual readability. By building the co-occurrence thing into the algorithm, google required "marketers" to up their game as far as the copy they used on their site.

If concepts are too similar when grouping keywords what do I do ?
I will summarise and then ty to explain : When I group keyword with the keyword explorer if concepts (grouping keywords) that I find are too similar to each other how do I go about it... Ldg me know explain. When google doesn't have enough data to rank a website with content does it only use links ? Let me give you an example. If I take "title tag" as a keyword people have a lots of questions that can be answered which will create numerous concepts. However, let's take "hiking tours Italy" for example. If I go through the keyword explorer or all the other tools existing out there people don't have questions. All I find are variations or synonyms of my keyword and I group by low lexical similarity all the words are going to be very similar. I know that each variation can be grouped and considered a different concepts but writing about various concepts that mean exactly the same thing because of lack of data not very smart and useful... So in that case what does google do, does it rank a website only based on links or schema ? see that it has no way to rank it based on good quality content because of a lack of data (questions that could be answers that would lead to concepts). Sudden inexplicable drop in important keywords
I have been running around in small circles trying to work out why, after a year of careful SEO my companies site has suddenly dropped for all it's main keywords out of the first page to the second or third in our weekly report. One theory was that five backlinks with a spam rating of 10 might have had an impact - these have been removed, but I have been advised that these may not have been the cause. Our crawl diagnostics remain squeaky clean. I add one, carefully checked and optimized (as per Moz recommendations) blog posts once a week and nothing has been changed other than that. We have slowly and carefully built up a good backlink profile, but as well as the drop in rankings our weekly report also showed that we lost 109 backlinks from the previous week. I have not been able to find out what I could possibly have done wrong! I've checked all our crawl reports and webmaster tools and nothing has been flagged up. I did discover, whilst typing in some longtail keyword phrases that our preview site URLs as well as our live site URLs appeared in SERPs. I checked with our web agency who assured me that this was fine, but that they had put a redirect on these yesterday. I thought this was a bit strange as that seems odd as we have been live for a year. I guess my question is could the fact that these redirects have not been active (until yesterday) have any bearing on what has happened or is this completely unrelated? Should we go after this main keyword?
Hello, We run an online store. The main content keyword for our niche is very competitive, but if I was going to go look up information and I was one of our customers, that is exactly what I would type in - this main general keyword. We have an expert in the field to write it and plenty of time. Although the main keyword is competitive, there are many many subkeywords that are a lot less competitive that would be answered in the article. It's tough to find good topics in this niche. We're thinking about doing a "Complete Guide to X". We would have far less backlinks and authority for about half of the 30 keywords it will cover than our main competitors. Should we do this and spend the next couple of years working on it, or should we perhaps target a smaller topic? Any advice is appreciated.

Title Length Vs Keywords
Hello all, I've been talking with an SEO expert who convinced me to add more keywords to my titles of a section of our site which is updated with products daily. I can see the logic and I do prefer having these additional keywords. The problem now is in Moz it says we have over 2,000 pages with title elements that are too long, which is true they are all over the 70 character limit. Is this a problem SEO wise? Speaking to our SEO expert they said it's not ideal from a user point of view as you can't see the full title, but are we going to be upsetting Google by having 150+ character titles? Thanks!

Bolding Keywords
A client has just switched from us to another provider and I have been checking up on the work done and the only change they have made is to bold lots of keywords on each page - I thought this was a practice that did not work - is there any evidence of this working or not working? Any articles/proof that we are not using out dated practices as we stopped doing this ages ago and yet the new provider is doing this. Who is right or wrong?

Meta Keywords: Should we use them or not?
I am working through our site and see that meta keywords are being used heavily and unnecessarily. Each of our info pages will have 2 or 3 keyword phrases built into them. Should we just duplicate the keyword phrases into the meta keyword field, should put in additional keywords beyond or not use it at all? Thoughts and opinions appreciated

Price Range: Top Notch Keyword Research
How many years would someone need to have to be experienced enough to provide a thorough job of providing me with keyword research? I am willing to pay for the best job but I don't want to be taken to the cleaners...especially by someone that may not be giving me the best effort or that is not qualified. ( I was just quoted $200 an hour from an 18 yr old today) We are getting ready to change our URL's and add keywords but I am not sure which terms to add and in what order. I have heard put the most important words first but I am not sure which are the most important. 🙂 By my research we only have about 5 keywords but I am not sure. (Excluding long tail)
