Meta Title and Description for click through optimization

I have a few years experience optimizing PPC text for click-through and conversion rates. I don't see many web sites that are using these methods for their organic listings generated from meta tags. Why is this?
So many meta tags seem to be either keyword stuffed or auto generated. From my research, it seems that keywords in meta tags mean little to search engine optimization other than the fact that vertically aligning keywords from search phrase to listing to landing page is important. (Just like it is in PPC).
Now that I am personally doing more than just PPC, I have begun rewriting meta text for increased conversions. The search results look to me like My web site is the only one doing this.
Is everyone missing out on something, or am I missing something?

I think a lot of people fear making changes to Meta descriptions and page titles especially once they have established rankings for fear of rocking the boat! Whilst with PPC you're able to continually adjust and monitor, doing the same with organic listings is probably seen as a bit of a risk to many?
Having said that, I think it's probably a very under utilized method of conversion rate optimisation at a very early stage, so if you can effectively track it & get the mix of good CTR whilst holding/improving on rankings, then you're on to a very good thing IMO

Best Practices-Optimize Theme Before Coding or After Coding?
We purchased a Wordpress Real estate theme called wpcasa. My developer started coding it. We hired a new SEO. The SEO looked at a simple page (About Us) and saw about 70 Java scripts and 11 templates and was very dissatisfied with the coding and heavy source code contained in the theme. My SEO claims the optimizing this theme (removing the unnecessary Java Scripts and combining the templates is way too much work and that it would be better to start from scratch and purchase a theme like https://my.studiopress.com/themes/agentpress/ with more compact, efficient and streamlined code. Problem is we need to throw out 2 months and thousands of dollars worth of code. My SEO says that in the day of Google's "mobile first" streamlined code is particularly important. My developer has taken the position that the code can be cleaned up after the design. Where it seems my SEO wants clean code to stat with. I don't know who to is right. Any ideas? Keyword optimization when two keywords mean the same thing
How does one best go about optimizing a page, meta title, and meta description for two keyword when the two keywords mean the same thing (e.g. attorney vs. lawyer). It's awfully tedious, obnoxious, and spammy-looking to keep putting "attorney/lawyer," "attorney-lawyer," or "attorney or lawyer" into content, titles, and descriptions.

Should i not use hyphens in web page titles? Google Penalty for hyphens?
all the page titles in my site have hyphens between the words like this: http://texas.com/texas-plumbers.html I have seen tests where hyphenated domain names ranked lower than non hyphenated domain names. Does this mean my pages are being penalized for hyphens or is this only in the domain that it is penalized? If I create new pages should I not use hyphens in the page titles when there are two or more words in the title? If I changed all my page titles to eliminate the hyphens, I would lose all my rankings correct? My site is 12 years old and if I changed all these titles I'm guessing that each page would be thrown in the google sandbox for several months, is this true? Thanks mozzers!

Title tag on Google starts with company name then :
Can someone help me and tell me why Google picks up and shows the title tag as for example: SEOmoz**: SEO Software. Simplified.** Then if you click through and look at the cache version of the page it shows the title tags as just SEO Software. Simplified. How will engines deal with duplicate head elements e.g. title or canonicals?
Obviously duplicate content is never a good thing...on separate URL's. Question is, how will the engines deal with duplicate meta tags on the same page. Example Head Tag: <title>Example Title - #1</title> <title>Example Title - #2</title> My assumption is that Google (and others) will take the first instance of the tag, such that "Example Title - #1" and canonical = "http://www.example.com" would be considered for ranking purposes while the others are disregarded. My assumption is based on how SE's deal with duplicate links on a page. Is this a correct assumption? We're building a CMS-like service that will allow our SEO team to change head tag content on the fly. The easiest solution, from a dev perspective, is to simply place new/updated content above the preexisting elements. I'm trying to validate/invalidate the approach. Thanks in advance.

Getting tons of duplicate content and title errors on my asp.net shopping cart, is there way to resolve this?
The problem I am having is that the web crawlers are seeing all my category pages as the same page thus creating duplicate content and duplicate title errors. At this time I have 270 of these critical errors to deal with. Here is an example: http://www.baysidejewelry.com/category/1-necklaces.aspx http://www.baysidejewelry.com/category/1-necklaces.aspx?pageindex=1 http://www.baysidejewelry.com/category/1-necklaces.aspx?pageindex=2 http://www.baysidejewelry.com/category/1-necklaces.aspx?pageindex=3 http://www.baysidejewelry.com/category/1-necklaces.aspx?pageindex=4 All of these pages are see as the same exact page by the crawlers. Because these pages are generated by a SQL database I don't have a way I know of to fix it.

What is the optimal URL Structure for Internal Pages
Is it more SEO friendly to have an internal page URL structure that reads like www.smithlawfirm.com/personal-injury/car-accidents or www.smithlawfirm.com/personal-injury-car-accidents? The former structure has the benefit of showing Google all the sub-categories under personal injury; the later the benefit of a flatter structure. Thanks
