Starting over from scratch makes sense if you've dug yourself in so deep a hole (say tens of thousands of spammy links) that it would be easier to rebuild your link profile from scratch, rather than going through the effort of deleting/disavowing all your bad links. Links from sister is a little trickier, but it's pretty standard practice to link to sister sites from the sidebar or footer. If you're worried about Penguin, you can keep the link to just the homepage or about page.

Re-writing so many product descriptions is a big task. I would first see if you can combine any of the products into a single page.

Like others have said, there is nothing wrong with reciprocal links from other sites as long as they're relevant. The one thing to watch out for is sitewide links, especially if you use keywords as your anchor text. That can get you hit by Penguin. Best to set up a separate "resources" or "partners" page, and avoid using your keywords for the anchor (use brand name or URL).

domain.com/blog is probably the best option, since Google often treats subdomains (blog.domain.com) as separate sites. By having your blog in a subdirectory, any link value it accrues will benefit your main site as well.

Remember that every keyword you add to your URL decreases the value of every other keyword you have in the URL. You want to include a few keywords in your URL for SEO value, but not so many that it dilutes the value of the other keywords. Also, having an overly long URL is a poor user experience.

The discrepancy between noindexed/indexed pages is not in itself a problem. However having all those pages will present a challenge to Google, in terms of crawling. Even though the pages won't be indexed, Google will need to spend some of your limited crawl budget crawling all those pages.

Just because you create a new page and delete the old one, Google won't know immediately about it. So if Google crawls the new page before it's had a chance to crawl the old one, then it will indeed consider the new page to be duplicate content. Eventually it may de-index the old page if it keeps returning an error.

If you are appending the city name to the keyword, then it's not a duplicate title. There must be other pages somewhere on your site that are generating the duplicate titles. If you are using Google Webmaster Tools, it will show you all the duplicate titles on your site as well as which pages are using the same title.

Wordpress can create a lot of duplicate content issues because it generates things like tag pages, category pages, and archive pages automatically. No-indexing most of these is a good idea, unless you are going to spend time creating unique content for them.

My personal preference when it comes to navigation is fewer options than too many. The tendency is to want to put everything in there, "just in case", but most users will not even click on a fraction of those links, and you can end up confusing people with too many options.
