Page Optimization score is An A but Rank is dropping

New to company in SEO and they wanted a page to rank high for certain term : knowledge management on this page: http://www.apqc.org/knowledge-management
Ran SEO Moz optimizer and did suggested changes. Moved score to A and ranking was on google first page but now it's dropped considerably last 2 months as the bounce rate has gone through the roof.
I'm at my wits end and don't want the higher ups on me for the drop. I'm not sure what to try next.

Your welcome - I would think having the word comments without no comments on every page can be a red flag to Google that your trying to game them and get undeserving long tail keyword traffic with the word comments in it.

The H1 nested in the breadcrumb will disappear when we launch the site redesign in three weeks so I figured best to not ask IT to do the site wide fix when it will happen anyway.
The comments feature on our site is only used on the blog so is that something I should think about removing from most pages because it's affect on SEO is either bad or neutral at best?

Thanks and you made a great point as well. The links to this page should be done in moderation and avoid multiple links on every page to the KM page

You could also just spell out Knowledge Management instead of using KM. I'm sure google doesn't know what that is. You're also using the KM in a lot of your anchor text, It may benefit you more If you spell it out so google know's that you are pointing at something relevant.
And like Irving said, use some schema to point to different things, like the book at the bottom of the page.

Here are a few blatant issues:
You have your H1 tag nested into the breadcrumb. That's not acceptable.. Use the breadcrumb microformat
This is interesting, your text of knowledge management on the home page (and in other areas of your site) links to a different page! You're giving Google mixed signals on which page is the the knowledge management authority on your site.
Your video should be tagged with the schema.org video object tags
Why do you have the words COMMENTS in big red text on bottom of the page. (just looks odd, so I had to mention it.)

Strange landing page in Google Analytics
Hello MOZ Community, The website in question is https://x-y.com/ When i looked at the landing pages report in GA , x-y.com is appended at the end of every URL like this. https://x-y.com/x-y.com When i open the above URL in GA interface, it shows page not found. Changed URL's, traffic dropped from 2k week to 1K week. Need advice!
Hi Mozers, I recently changed my URLs for my ecommerce site and my traffic went from 2,000 visitors a week to 1,000 visitors a week, over a 3 week period. Traffic is down, so are unique Kwds. I need advice on why this happened and what I should do moving forward. To brief, I have a ecommerce website, www.ecustomfinishes.com. I noticed pattern that a lot of my URLs with a unique URL structure (URL.Com/ProductDescription/ProductName) were getting a lot of entrances ~30-50 a month, and others that followed the path of my subcategory (URL.com/SubCat/Product) were getting 0-3 entrances a month. The seo pattern was that those with unique product URLs were hitting long tail Kwds, and those URLs with /subcategory/product were getting far less traffic. I changed 150 or so urls to be unique. Good idea, I thought. Since then: CON: Since then my traffic dropped from 2200 visitors a week to 1100 visitors a week. -25% week to week, over 3 weeks CON: # of non-paid keywords sending visits: -25% week to week, over 3 weeks PRO: my Urls receiving entrances +10% week to week, over 3 weeks REF: http://imgur.com/GwZT8 Question: What are your best suggestions moving forward? Traffic dropped 75% - Panda 2 Penalized?
Hello everyone, My e-commerce website traffic dropped by 75% overnight, on April 12th. The website has been around since 2003 and growing every year. Over the years gained some great placement for some competitive keywords and stayed on page 1 for some time. After the Panda update, some of our major keywords dropped from page 1 to page 10. The first Panda update had a slightly negative effect on us, by bumping up all the brand's websites (brands we carry) placing the brands in the first couple positions (giving them 2-3 first spots) pushing us down. However, the Panda 2 killed us. I was wondering if anyone could shed some light on my situation. Did anyone ever have luck or would recommend contacting Google, and asking them for advise or reasons why the site was dropped. The website in question is www.instyleswimwear.com Thanks in advance,

Ranking Testing
At times when I change something on my site it effects my rankings. For my business I care mostly about my rankings in certain locations (not local). How can I see how I am ranking in various locations at a given time? I see several tools out there, but none of them seem to work. For example I recently I added a badge to my site and my ranking dropped a spot locally, but I do not think it did elsewhere. The value of the badge is worth that if it is just local, but if it made me drop a ranking in one of my major markets it would not be worth it.
