Can't seem to rank for keyword "home care grand rapids" - need some advice

I am trying to rank for "home care grand rapids" and am having a really difficult time. My site: http://healthcareassociates.net has better backlinks, keywords and other seo markers than my competitors but I still can't seem to rank.
The keyword and associated keywords (home care grand rapids michigan, home health care grand rapids, etc.) are only 31-33% difficulty and my site/page rank is better than the leading sites. What gives?

I am trying to change the Title tag in the settings of Wordpress and it says, "Home Care Grand Rapids Michigan - Health Care Associates". I don't know why the extra "Home" tag is automatically being put in there or how to remove it.

I don't do local search because all of my sites compete nationally.
If I needed my sites to rank for local search I would hire Miriam Ellis, Moz member and Moz associate (if she is available) to help me get set up properly and begin promotion. I don't think that the cost would be that high and I would have it done quickly, properly and efficiently. This is too important to take the chance of an improper set-up - at least that is my opinion. This could probably be done via gotomypc or other computer sharing - so you can see what is being done.

Thanks so much for the info. Maybe I am using Open Site Explorer wrong. I type in "home care grand rapids" in Google and pull off the top 4 pages and put them in OSE. Then I look at the results and see that my site is 2nd in page authority, 3rd in total links, 1st in total linking root domains, etc. Seems like it is doing well compared to the others.
Also, you say to get good references. For a different fitness site I had I could easily find other bloggers to guest post for but in the home health industry it seems like there aren't any bloggers. How do you get quality back links when there aren't other bloggers?

I appreciate the recommendations and will definitely make them.
What else would you do for local search? I have secured and verified my Google place page and other business listings and am working on getting reviews for them. Any other ideas?

Hey Lori,
It seems you're on ranking on the second page of results on Google. Except one or two, all your competitors have better metrics including Domain Authority, Linking Root Domains and number of links. You'll just need to keep getting good references and citations, and you'll slowly move up the ladder.

I believe that google is seeing the first instance of "Home" and associating that with "Homepage".

THEN.... get your target query "Home Care in Grand Rapids" in the text of your homepage - preferably in the prominent text at the top of the page.

This will be an improvement but I doubt that it will be enough to fix your problem. I don't think that your links are competitive.

If I owned this website I would put my big effort into local search.

Hello, I have a client that I have been working on their primary site for the last year or so. In the last month they decided to have one of their internal employees setup a small shopify store. Now they are asking for the analytics tracking codes for it. On Google Analytics, Pages that were 301 redirected are still being crawled. What's the issue here?
URL that we redirected are being crawled on Google Analytics. Since they dont exist, they have high bounce rates. What can the issue be?

What's the best way to figure out which keywords are the highest converting?
We have a client using Google Analytics. They currently have 3 goals set up to track when website visitors fill out 3 forms: Form A, Form B, Form C. I can easily figure out what traffic sources have driven the highest number of conversions on each form (Search for Form A, for instance, or Referrals for Form B), but of course, when I try to drill down on search terms that have driven conversions to each form, I get stuck in "not provided" territory. I'd like to know what people are searching for when they ultimately fill out each form. This will answer questions like: are people familiar with us already when they convert, or did they randomly find our website when searching for something we sell? It seems like there must be a way, using Google Webmaster Tools, Analytics, or another third-party app, to answer the question: what keyword searches are responsible for the highest number of conversions? Especially on a website that has traffic of 10,000+/month and a healthy dose of search traffic. Right? Where am I missing this information?

Any advice for better results for local search in the UK ?
Hi there, I have tried the tools here, and they are BRILLIANT but maybe to much for me. I just wondered if anyone in the UK had time to give me some advice. A lot of my customers are small businesses (mainly trades, like plumbers, builders etc etc), and I am trying to find best practices for getting them more quality traffic amidst a lot of competition. One customer I am working with currently has 50+ competitiors for his small plumbing business that are within 5 miles of him and have an effective web prescence. In those circumstances (take it as read that I have killed myself creating quality content that I am proud of for this client), does anyone have any killer advice for me that could make my sites more effective ? My Domain Authority Dropped Dramatically..Can Someone Tell Me Why??
Over the last month or so my Domain Authority, my MozRank and MozTrust have all dropped off considerably. I haven't been doing anything different. How to Setup New Domain in Webmaster Tools? Can Anybody HELP?
I, I have a new domain, and a new account is now created in webmatser tools. Is there anythings I should know about using or setting up webmaster tools? For example, the geographic target, I'm in Canada our web site, a local Pilates studio, is a commercial web site to provides informations about our services. Should I select Canada? One thing about this, I have another domain, http://www.furnacefilterscanada.com/ and in the setiing, I have select geographic target to: Canada. When using google.ca result are better then when using google.com!
