Is my 404 page set up correctly?

HI there! In using a few different tools, and I have received varied result concerning my 404 page. In WebSite Auditor from Link Assistant, it says my page is set up incorrectly. My webmaster has told me that it is set up correctly, but at this point I'm not entirely sure what's what. Is there another way to test it?
I guess i'm not quite sure about the jargon and what "return 404 response code" actually means.

To add to Patrick's great response, this page describes what you are seeing. When you try to go to a page on your site that does not exist, you seem to correctly get an error page.
But the content of the page does not necessarily reflect the HTTP response returned by the server. Your server is saying that the page was found, although the content of the page says it wasn't found. [A 404 response from the server simply means Not Found.]
If you look in your Google Webmaster Tools under Crawl > Crawl Errors you will see a tab called Soft 404. A soft 404 is a page that says it is not found but is not returning an actual 404, not found, response.
[I always wondered how Google could tell--I was given the answer that if Google sees that a whole bunch of different URLs bring up the same page, which does not return a 404 response, it assumes that that page says it is an error page and labels it a soft 404.]
If you want to test whether a URL on your site is correctly returning a 404, you can use Fetch as Google to see if it returns Not Found or else there are a number of free server response checkers online.

Thanks! Currently, this page is returning a 200 status code, meaning that it's OK.
You need to change this status code to a 404 status code in order for it to return a 404 to crawlers.
I see your site is running on Apache - here's a resource on how to return a 404.

New pages on my web site
I have created web sites that appear somewhere on Google in hardly any time at all, but I appear to have forgotten something or things are different for pages added recently to an existing website. I have added a page on a particular subject, optimized it using on page grader, so that I get an A, and a check mark for everything except H1 tags and rel=canonical which my web hosting provider does not support. I do have a check mark for accessible to search engines The page has the format http://www.domain.com/specific-keyword It is in the menu, so should have internal links to it, as I understand it. I have created a new site map, and submitted it in webmaster tools. Interestingly it says that of the 96 pages only 76 were indexed is this a clue? and why would they not index a page I have then shared the page on google plus, facebook, tumblr, pinterest and twitter and some others In OSE it comes up as domain authority 28 page authority 1, the social media shares do show up in metrics on the right but no links internal or external are shown, they do on other pages I created in the same way. Is it just a case of waiting or is their something I do to help thank you

Why is moz saying I have a 404 error?
I recently asked a question on here about how to fix my 404 error. I figured it out, and now webmaster tools says it's not there any more. This as been confirmed by keeping watch for about a week now. But Moz just crawled my site today and is for some reason showing an error. Am i missing something here or is Moz a little slow sometimes?

Confused on setting up my domain in a campaign
I have a website that is www.example.com. When I first setup my campaign in SEOMoz, I missed the instructions that stated I need to set up my domain as it appeared in the browser. Thus, I set it up as example.com. When I realized my mistake, I setup a second campaign as www.example.com and am running them side by side to see the differences. My question has to do with the best setup. I have a blog that is a subdomain as blog.example.com. It appears that the www.example.com campaign is not crawling the blog pages but example.com is. Are there any downsides in setting up this campaign as example.com? I have a number of other sites with the same issue so would like to get this resolved before I setup the additional sites. Thanks for any assistance or insights into ways I may be "shooting myself in the foot" with the wrong domain structure in my campaign settings.

Why are not nofollowed links counted in On-Page Analysis Report?
When I run the On-Page Analysis on our homepage, the report says the page has 238 **"Internal followed links". ** Why are not nofollowed internal links counted as well? Nofollowed links have been leaking link juice for quite some time now. Martin

SEOmoz tools: Keyword Difficuty + On-page Analysis
To analyse the current KWD situation. I would want to see the top 10 results link metrics (as in Keyword Difficulty & SERP Analysis) and each page's On page score (as in On-page Analysis) for the keyword. Those two figures would give me a pretty good picture of the current situation. Kind regards,
