Google adding strings to meta title in SERPs => Driving my client crazy!

Fellow Mozzers,
I'm really baffled by Google's behaviour adding strings before my meta title in the SERPs.
This behaviour seems random, but it mainly happens on queries that trigger the homepages (multilingual .com domain).
I'm aware of meta title guidelines concerning length, and I know that Google may insert strings if it thinks it could benefit the user.
But clearly their "aid" is not helpful at all...Client is really anxious to get this resolved and we're stumped on what could be the cause for this...
Some examples:
Query: https://www.google.co.uk/#hl=en&pws=0&q=Ogone - Online payment gateway
Result title: en-gb - Ogone - Online payment gateway
Normal title: Ogone - Online payment gatewayQuery: https://www.google.at/#hl=de&q=Ogone Online Payment Service Provider
Result title: ogone.at - Ogone. Online Payment Service Provider & e-payment ...
Normal title: Ogone. Online Payment Service Provider & e-payment Deutschland
=> For longer titles it will just prepend strings, and leave out our piece of the meta titleQuery: https://www.google.nl/#hl=nl&pws=0&q=Support Ogone
Result: Support | Ogone
=> For very short titles, Google doesn't add anything, why does it so for the other results?I really don't understand why this happens. The titles are long enough (but not too long), no strange characters inserted.
Please, please help.

We've found the solution. We needed make each meta title unique, so we added the country dynamicall to each meta title.

Yes we are using rel alternate to its fullest extent. But this still doesn't explain this strange behaviour.
Any ideas?

Are you using the advantage of rel="alternate" hreflang="x"?

Why Dropping pages from SERPS?
Our website for my olansi company in London, China has hundreds of pages dedicated to every service we provide to China local areas. The total number of pages is approximately 100. Google caters pretty well for long-tail searches when it indexes all these pages, so we usually get a fair amount of traffic when this happens. Prefix added to Titles in SERP
We are getting different titles in Search Engine Result Page, where PakWheels: is added to the actual title of page. Your expert opinion is highly appreciated. What can be the possible reason of "PakWheels:" appearing. This is a case for only home page of website. Search query 'cars in pakistan' shows 3rd result with title Pakwheels: Cars, Used Cars, New Cars, Latest Car Prices & News ... Whereas the actual title of page is without 'PakWheels:'

Does Google index has expiration?
Hi, I have this in mind and I think you can help me. Suppose that I have a pagin something like this: www.mysite.com/politics where I have a list of the current month news. Great, everytime the bot check this url, index the links that are there. What happens next month, all that link are not visible anymore by the user unless he search in a search box or google. Does google keep those links? The current month google check that those links are there, but next month are not, but they are alive. So, my question is, Does google keep this links for ever if they are alive but nowhere in the site (the bot not find them anymore but they work)? Thanks

Duplicate Title/Meta Descriptions
Hi I had some error messages in the webmaster account, stating I had duplicate title/meta descriptions. Ive since fixed it, typically how long does it take for a full crawl if Ive fixed these issues? Webmaster is still showing problems with various Title/Descriptions. Also was wondering if I should block individual pages on a large ecomerce site? EX of a Large site - http://www.stubhub.com/chicago-bears-tickets/ (Page is structure and optimized) Then you have all individual games http://www.stubhub.com/chicago-bears-tickets/bears-vs-lions-soldier-field-4077064/ they have an H1 and a meta description, should the page above be blocked from google and concentrate only on the Pain page? Sitemaps for Google
In Google Webmaster Central, if a URL is reported in your site map as 404 (Not found), I'm assuming Google will automatically clean it up and that the next time we generate a sitemap, it won't include the 404 URL. Is this true? Do we need to comb through our sitemap files and remove the 404 pages Google finds, our will it "automagically" be cleaned up by Google's next crawl of our site?
