Multiple business location - a nightmare scenario

Hi Guys,
I was wondering how should i tackle the url structure for the following scenario:
Company provides several hypnotherapy services including 'stop smoking', 'cbt', 'weight loss' and more ... and have offices in 3 cities.
Currently, the website has the following structure.
www.website.com/stop-smoking-city1
www.website.com/stop-smoking-city2
www.website.com/stop-smoking-city3and so on ...
would it be better to have the following url structure ...
www.website.com/city1/stop-smoking
www.website.com/city1/weight-loss
www.website.com/city1/cbtand so on ...
Please advise!

Does that mean that we will have to write the services page specifically for each city multiple times? So as to say ... stop smoking article for 4 different cities ...
Another approach i was thinking of was to have page on stop smoking have links on that page where these services can be bought ... so on the right panel, we can say that these services are available in the following cities.

website.com/city3/cbtThis site architecture essentially tells Google "this is our brand. we have 3 cities. These are the services in each city."
The way you're currently doing it says "this is our brand. we have this service in this city." It's not as clean or organised.

What to do with multiple forms and thank you pages
Hi Everyone, I'm wondering what to do with form and thank you pages. I asked a question a long time ago about the contact page as noindex and was told by people that its better to leave it and write content for it and thats what we did. Now I have a client that does self storage and they have 4 locations and each location has a reservation page with a basic form but no content. Each page also redirects to a thank you page with tracking codes. There's a total of 6 "thank you" pages with different codes (this was done by yellow book). 4 "reserve your storage pages", 2 pages to pay for storage with iframes to 3rd party payment portals. I was told to noindex these pages but I'm not sure so I'm asking here. I was also told to nofollow and remove them from sitemaps. Thanks Aron

How will it effect SEO to have multiple h1 tags on a page?
I have a client who recieved this advice from his marketing consultant: "If there are multiple h1 tags on a page, this can confuse Google and it may have a negative impact on the keyword rankings. If you could ask your web developer to go in and remove the h1 tags on the header images that would be helpful. This way it will be easier for Google to index your site and will help your keyword rankings." How will it effect SEO to have multiple h1 tags on a page?

Should Ltd be added to business names in page titles or not?

What is the best way to handle small business site architecture?
I do allot of work for small businesses with around 15-20 total pages. What is the best site architecture? For example if its a landscaping website, should there be a services category page and pages under that (domain.com/services/lawn-mowing.html) or should it be flatter (domain.com/lawn-mowing.html) They offer about 10 different services.
