I can't replicate this in live SERPs, but Matthew's explanation seems to match the evidence. It does appear that Google is doing some kind of implicit canonicalization. It's rare, but it does happen.
You could try giving the PDF a unique title in its own meta information (the document doesn't seem to have any). It's more likely to look like a duplicate if that's undefined.
The next step would be to recreate the PDF slightly and make it unique, but honestly, each step is going to get to be progressively more work. It really depends whether this is just an anomaly or it's causing you actual issues as a business.

I would say this is a duplicate content issue.
Identical versions of the PDF are found on both the sweetwater.com domain and also americanmusical.com:

Hmm.... This is wacky.
It looks like although it says "Sweetwater" in the Title Tag, the pdf file is still being hosted on your URL (as well as the actual rebate form having American Music listed as the return address).
Maybe perhaps whoever added this rebate form to your website "borrowed" it from Sweetwater? And by that I mean, copied over the pdf file, changed the address, and copied everything else (Title Tag included), but forgot to take off the Sweetwater brand name?
Either way, it's on your own URL and it's a rebate form that includes your own address. Just get into your site editor and change the title tag and you should be fine.

Can I reuse meta description tag on another site?
Hi....lets say i sell widgets and on my site i have redWidgets, blueWidgets and greenWidgets. Each of these widget page's has a meta description tag that seems to help click through. I now have a different site that would like to do a right up about each widget and will dedicate a independent page to each widget. Can i safely provide them with the original meta descriptions (from my original site) for each widget i have on my site to then use on these "right up" pages (as they seem to work well and are informative of the subject matter). Javascript(0) extension causing an excess of 404's
For some reason I am getting a duplicate version of my urls with /javascript(0) at the end. These are creating an abundance of 404 errors. I know I am not supposed to block JS files so what is the best way to block these? Rich Snippet Not Appearing? Why?
I run a price comparison website and am trying to get merchant name, product name and price appearing in SERPS I have used the Google Structured Data tool and after initally receiving some errors, I was able to remove the errors and ended up with what I thought would be my ticket for awesome looking SERPS. Using Robots Meta Tag on Review Form Pages
I have gone over this so many times and I just can't seem to get it straight and hope someone can help me out with a couple of questions: Right now, on my dynamically created pages created by filters (located on the category pages) I am using rel""canonical" to point them to their respective category page. Should I also use the robots meta tag as well? Similarly, each product I have on my site has a review form on it and thus is getting indexed by Google. I have placed the same canonical tag on them as well pointing them to the page with the review form on it. In the past I used robots.txt to block google from the review pages but this didn't really do much. Should I be using the robots meta tag on these pages as well? If I used the robots meta tag should I noindex,nofollow? Thanks in advance, Jake

Meta Keywords
Hello Everyone- Quick question about meta keywords. Most SEO's agree that meta keywords are no longer used as a ranking factor in Google.
