Selection of the Right Keywords - Some insights needed!

I have recently begun with my content and keyword selections.
I used the Adword's keyword tool and for eg: got a few keywords like
laser skin treatment
dermatology
dermatologists
cosmetic laser surgery
The competition for these are low and medium. Now what I understand is. If I wish to use them in my articles on content generation I can have them as medium and long tail keywords to write around.
So for eg: laser skin treatment = " Benefits of a laser skin treatment in India" and the url for this article could be /laser-skin-treatment
For dermatology = "best dermatology practices" url could be /dermatology-practices
Do the above happen to be the medium and long tail keywords? Am I going in the correct direction. How do I judge and come out with medium and long tail keywords. Please suggest

I didn’t see any problem with the idea. But long tail or short tail keywords are not defied on basis of its competition but because of the length of the keyword.
I will consider ‘dermatology’ as a short tail keyword with low or medium competition. Keywords like ‘Laser Skin treatment drawbacks’ can be considered as a long tail keyword.
Adding the keyword in the title and still works (may be not as effective as it use to be) but I still prefer people using it as it tells more about what the page is all about.

With the Panda update things have changed. Some of these practices are determined to be not quite as effective as they used to be. The key in optimizing your keywords is to make sure that at least your title and main header title contains the keywords. From there want to disperse your keyword usage. Yes try and avoid direct exact keyword over optimization so stick to other similar words that will best describe your content.
Also in Google keyword tools make sure you check "Only show ideas closely related to my search terms" and "[Exact]" to make sure you want these keywords.

Keyword cannibalization or linking structure?
Hi all, Recently I got an answer from this community about "why our login page is ranking but not my homepage for primary keyword"? Possibilities are keyword cannibalization or linking structure. In our case, our homepage is not ranking for "primary keyword" but ranking for other keywords. If it is linking structure, what might be wrong? Ecommerce or E-commerce as a Keyword?

Should I remove my keyword meta?
So it's safe to assume keywords are no longer used by SEs in the old fashioned sense to rank sites, but should be keep them as indicators of site content? It's been suggested by some that they're detrimental for two reasons: 1. Your competitors can snoop the keywords you're targeting but mainly... 2. Over-optimisation is the enemy these days! Does Word Order Matter in Local Keywords?
We do a lot of local SEO, and we're wondering if it's better to target "keyword location" or "location keyword"? Does it affect ranking and keyword difficulty if we're trying to rank for "plumbing appleton" or "appleton plumbing"? Any insight would be great.

Troubleshooting Decline of Branded Keyword Searches
Hi, Over the past year, I have seen a huge change in the distribution of our organic keyword traffic. I'm trying to research why our branded keywords have gone down. Is This Keyword Stuffing/Spamming?
We are a custom patch company--we make patches for many different types of clients. I have a gallery of patches for almost every kind of client, and they all have their own pages. If I put navigation on the home page such as what I show below, will Google consider that to be too much? Boy Scout Patches | Motorcycle Patches | Fire Patches | Police Patches | Military Patches | Sports Patches | Business and Organization Patches | Paintball Patches | Scooter Patches | In Memory Patches They would all be links to different pages, and there would be literally 50-60 more! Would it be better to remove the word patches from all of the links? And then another question comes up: too many on-page links?

Why have I lost rankings for my top 10+ keywords
Hello, As of 04/24/11 I have lost rankings for my top 10+ keywords in an extremely competitive market. I was ranking on page 1 in Google for keywords like: iphone app reviews, ipad app reviews, iphone app videos, ipad app videos, and more. Since 04/24 I have fallen off the first 10 pages + in Google for all major keyword & keyword terms. I have identified that I have 1,359 articles from prmac, which contain links to customers who published press releases through prmac. We are in the process of removing these pages, but creating a list of urls to prove to Google the possibly offending blog posts are gone. Beyond that I am not sure what else to do, please help.
