Are on-page optimization tools becoming outdated?

So from what I've noticed, Google is using a lot of semantics in their algorithm. When it comes to using a tool to determine on-page, keyword/phrase factors, most on-page optimization tools check exact matches and give you a rating and suggestion based on that. What about synonyms and "natural", related terms?
My question is, are these tools becoming more inaccurate by not using semantic/synonym checks?

I am beginning to think that these factors as outline my @Journeyman actualyl can get you some penalty points. And end up reducing your overall ranking.
I think it is as Matt Cutts and Rand have being saying for ages. Write content for people not machines.

Keyword Usage in URL [not possible if you are targeting multiple keywords]
Keyword Usage in Image Alt Attribute [we should not be focusing on keywords only while creating alt tags for images]
Exact Keyword Usage in DocumentExact Keyword Usage in Page Title [not that important] -
I'm referring to on-page checkers that our clients are using to check for themselves (including SEOmoz's one), and it gets very difficult to explain to them why the tool isn't the most accurate. In the end it looks like we're not doing our job.

_I do not think you should be using on page tools for checking out keywords density and all that. No, these tools are obsolete long ago. But yes you can always use Screaming Frog for checking the following on page factors -
pages with duplicate meta data
Pages with double h1 tags
Pages with meta data
Pages with 301 and 404 response code_

If I use a similar H1 on two landing pages for my website will it impact my SEO negatively?
I've been doing quite a bit of research on keyword cannibalization and duplicate content. I have two pages with nearly the same H1. The rest of the body is different, with a few similar overlapping keywords and points. Am I right in that nealry the same keyword in H1 on two separate landing pages will damage by SEO?

Narrowing keywords or build more pages - Startup
I provide local services to a number of small cities and am trying to decide the best way to attack local keywords. I need to decide if I should target all 4 cities or narrow it down to 1 or 2. I used the google keyword tool and none of my keywords have enough results to show data so I am left guessing by using the nearest major city and population sizes. Anyway, I have read over and over about how keywords in the title / description are imperative. Well, thats tough to accomplish without being spammy when you offer two services for four cities. I don't think lakeville snow removal, apple valley snow removal, eagan snow removal makes for a good title of a page. Also, it's not an easy task to get any kind of keyword density in the content as well. So how do you recommend I attack this? I have seen sites that create a page for each keyword with basically the same content excpet replacing city x with city y and they do well. I find this spammy and hope that eventually they get penalized for it. I guess I would be willing to do it, but would prefer a more natural solution. One more question, if I do keyword a single city, say Lakeville, what is the prefered way to keyword the home page compared to the service pages. Example, I have a snow removal page that the keywords would be lakeville snow removal and a lawn care page with the same.. So what is the target for the home page? Best tool for drawing sitemaps?
Drawing up sitemaps is one of those tasks that seems to take me twice as long to do electronically that it does on paper. Unfortunately, sitemaps scribbled on sheets of A4 paper aren't much good for sending and sharing. So, Mozzers, what tool or software do you recommend for drawing up sitemaps quickly and easily? So far I've tried using Visio, Excel and Word but if there's a dedicated tool out there, that would be awesome. Thanks in advance, Cheers

Best Keyword Research Tool / Strategy / Approach?
Currently, I list every root word (one word) I can think of for the client. Then start a 2 word keyword section, making every possible connection with my root words. Then move to 3 word, 4 word, and so on. I do this all in a word document and by hand. Does anybody have ANYTHING (tool/strategy) that does something similar? I'm looking for anything that can expedite this process. My manual method makes me feel like I'm always forgetting certain combinations of words.
