Do You Add City Name & Key Word For Every Page?

Hello,
I'm new to SEO but feel I have a decent grasp on it. However, I had a question pertaining to key words and using my city name in it. For instance, if I am using the key word "herniated disc treatment" do I need to put in my city name behind it or does google recognize that I am already in my city area because of my geo tagging and having it listed on the footer of my site? Any help would be greatly appreciated. Thanks!

Donna has offered some good advice. Something else you need to consider:
If you have a single location, then yes, most of your content should be mentioning your city. Your Herniated Disc Treatment page can talk about how you treat this problem in your office in San Diego, and that's completely natural to do so. However, if your practice expands to more than one location, then you'll have to revisit this strategy. If you have 2 offices, or 10, or 100, how much city-oriented optimization you can do of your service pages will have to be rethought, because you're not going to want to put a list of 10 cities on your service pages.
But, for now, if you've got just one location, a moderate mention of your city terms on your service pages (and other page like Home, About, etc.) is totally fine. Just don't overdo it.

It's a good question and the answer depends on how much competition you're up against.
Yes Google recognizes your location using geo tags and footer content. I generally try to optimize without using too heavy a hand when it comes to city names. But if you're having trouble ranking using the more subtle tactics alone, then, by all means, employ the city name in your title tags, meta description, H1 headers, and so on. Just make sure your content doesn't start to look "stuffed" or unnatural as it may turn off visitors. It's most important to keep them interested and engaged.

New Page Not ranking?
One of this client's top keyword is "oak beams". They already rank well in the UK for other related terms like "reclaimed oak beams" at /reclaimed-oak-beams/ and "air dried oak beams" at /air-dried-oak-beams/ We have created a page at /oak-beams/ but this page ranks nowhere? Instead the reclaimed oak beams or air dried oak beams page ranks for the term "oak beams". Any ideas why Google is swapping between those pages and not choosing the /oak-beams/ page? A few notes are that the /oak-beams/ page is newest page on the site and yes I know there are no links pointing to it but there are no links pointing to the other pages either?

In my page title, I have my product name. Is it beneficial to also include another keyword like: Buy wedding dress online Australia: e..g. (page title) amelie wedding dress | buy wedding dress online Australia. Or is it better just using: Amelie wedding dress

Should we add our company's name in page title tag or not?
We have been adding our company (Townscript) name in all the page titles. For example, in an event page of Lucknow Conclave: www.townscript.com/lucknowconclave the page title is Lucknow Conclave | Alexis Society | Townscript I read somewhere that it's not necessary to put your company's name in the title tag. Is it right? Please help!

What's better for SEO a page per review or a page with all reviews?
Was wondering what's better for SEO. We have a platform where consumers can read and write reviews. But the question is: is it better to give one page per company with all the reviews on it? Or should we have different pages for the specific company? Example: Itunes has a company page with all reviews on the page, but not the whole review. You have to click further to view the whole review (new page), at the moment this the current situation. What if we place the whole reviews on the company page, so you don't have specific pages for the reviews? Hopefully can someone help us out. On page SEO Strategy / What pages to use?
What is the best page to use for targeting your hard to rank keywords? 3 keywords optomize for home page. Should I create page with thoses keywords or leave it like this?
My online store home page, Furnace Filters Canada has 3 keywords with good ranking in google.ca keywords: ''furnace filters canada'' rank #1 position in google.ca keywords: ''furnace filters'' and ''furnace filter'' are on 5 or 6th position of page 1 in google.ca Those keywords are bringing most of the traffic to our site. Is this a waist of time? If I decide to create a new page for optimization with, do I have to create one for singular and another one for plural? Creating a new page also mean removing, '' Furnace Filter'' in the home page title, until the new pages are index, I'm afraid to loss that 5th position in Google. Why has only one page been crawled?
