Why is On-Page showing canonical wrong?

I'm trying to use the On-Page report card and it's saying that my rel=canonical is wrong, I've looked and I can't see anything wrong with it, am I missing something?

try this generic code:

RewriteRule (.*) http://www.%{HTTP_HOST}/$1 [R=301,L]Alternatively, try your code but lose the $ at the end of the first line like this:

Thanks I never noticed that, I don't have a clue how to fix it, if I use non-www it takes me to www which is what it should do but it doesn't seem to work if it's in a product, I've tried adding

I would take a look at how you're handling redirects. Any page that I remove the www from the URL takes you to the home page and not the www version of the page. That's a huge mistake. I would suggest fixing that right away.

Hey, I have just ran that page through the on page optimisation tool and it comes back with no errors, perfect score as it happens. Think it may have just been a bug when you ran it through or some other small problem as it certainly seems fine now.

It's in the on-page, as part of the critical factors, doesn't really give me a reason for the error it just says

I have read in other discussions that having all questions on an FAQ page is the way to go and then if the question has an answer worthy of its own page, you should abbreviate the answer and link to the page with more content. My question is when using some templates in WP, they have a little + button you can click and it reveal the answer to the question. Search Console shows structure keywords more significant over content keywords. What is wrong?
Search Console shows non content related - website structure keywords as:

"categories, account, facebook,..." as top significant keywords. And after those comes the website's content keywords.
These words come from the header as I can notice. Am I repeating the content keywords less? Is there a way to adjust this? I am sure this affects my Adsense targeting.

Why are my pages de-indexed?

It just happened today, so maybe I am jumping the gun but I doubt it. When I enter the page into google webmaster tools again and press fetch, one of the child pages, it re-indexes. What could be the problem here? I had someone re-write the content for every city but I have a feeling that there is less differences in the car accidents pages? Is it redundant to include a redirect to my canonical domain (www) in my .htaccess file since I already have the correct rel="canonical" in my header?
I've been reading the benefits of each practice, but not found anyone mentioning whether it's really necessary to do both? Personally I try to stay clear of .htaccess rewrites unless it's absolutely necessary, since because I've read they can slow down a website.
