Why are my pages de-indexed?

Hello all,
I am very new to SEO.
For some reason many of the pages on my site were de-indexed.
Specifically the ones linked from this page:
However other pages, like the ones linked from this page and this page were not de-indexed.

However the pages linked from this page were not de-indexed: http://www.lawyerconnection.ca/practice-areas/slip-and-fall-lawyers/[2]

That first page itself was not de-indexed, just the site that it links to.
It just happened today, so maybe I am jumping the gun but I doubt it.
When I enter the page into google webmaster tools again and press fetch, one of the child pages, it re-indexes.
What could be the problem here? I had someone re-write the content for every city but I have a feeling that there is less differences in the car accidents pages?
Is this considered duplicated content do you think? Am I making some other mistake I can't think of? Is it just a one day blip (I doubt it)

Hmm do you see the ones that I said look like they've been de-indexed? I think they were! Why would that be?

Over Optimization may appear to google as an attempt to rank for terms by spamming or repeating unnecessary words. That is how in the past online companies ranked easily.
Google has changed the way their algorithm reviews sites and they look for spamming or over use of keyword phrases.
Now no-one is suggesting you are doing it it on purpose as it often happens by accident. Hence all the URL's that Patrick has highlighted appear to have that ominous "over optimization" look. The URL's are all the same except the city or the town at the end of the URL.
If still a bit muddy ask away happy to clear up, a good question...

So here's why I think they're de-indexed. I'm tracking google rankings and all my "Car accident lawyer-City" totally disappeared today.
If I go to google and I search:

http://www.lawyerconnection.ca/car-accident-injury-lawyers-kingston/
It doesn't come up. Yesterday it definitely did. This is why I think it's de-indexed.
Also my impression was I couldn't do Google My Business for all the different places because I don't have have brick and mortar locations in all of those cities.

I am not exactly sure what you're asking here. According to my search, your site is being indexed, including the pages that you mentioned above. 164 pages out of the 168 I was able to crawl, in fact.

I would take advantage of listing opportunities through Moz Local, Whitespark, and Google My Business if you have multiple practices. There are also service area opportunities available through Google.
Have you checked to see if you have a manual action of any kind? Have you seen your organic traffic at all?
I would take a look at geo-targeting your site as well in Google Search Console.

Moz Academy There are a ton of learning resources above that will help you get off on the right foot with SEO. Right now - I would focus on your on-site SEO, information architecture, and also your content. You have a lot of room for improvement on your website, but with proper research of your audience and keywords, you will be off in no time.Again, I don't see any reason why you may feel you've been removed from the index, but I am definitely not seeing a whole lot of reasons for your site to rank either. Not that's not a knock against you at all, there is just a lot of work to be done!

Inserting Keywords in Web Pages
Hello! Question: When I add chosen keywords to my site (in urls, title tags, meta description, page content, headers, etc), do I need to put the words on my webpage exactly as they appear in my keyword research? So if I searched "therapist bay area" and I want to include these keywords, for example, must I use those words in that order in a sentence or header on my homepage? Renamed a page and created a 301, page lost its rankings.
We changed a page name to fall under the root of our site from domain.com/page1/page301d/ to domain.com/page301d/ and after 2 weeks it still is not back to its #3 position. Now it is on the bottom of page 3. I cant figure out what im doing wrong here. The original .com/page1/ that this page fell under was removed totally and redirected to antoher page that was more relevant. I went ahead and re-enabled this page and its contnent, because the page was linking out to the page we 301d. This page we re-enabled had about 150 links poitning to it and therefore i was thinking that maybe the link juice from this page (or relevancy) via an internal link was helping it rank. This was updated about 6 days ago and the internal link is back Any other ideas why this might not be working. Ive checked all the 301s, content has not changed on the page. We have updated the strcuture for many pages. Instead of having the pages in question fall under anotehr page, they all fall under the root and its sub content is now only 2 levels deep , instead of being 3. hope that makese sense.

Will pushing a visitor to a conversion page hosted on a 3rd-party domain hurt the landing page ranking
Had an interesting question from a client. The client has a page that is optimized for a specific term. The goal of the page is to push users to sign-up for a trial. The trial registration (conversion) page is hosted by a third-party. Will pushing users to the conversion page cannibalize the SEO authority of the landing page. My reflexive answer is to say no, but now am not so sure.

Wordpress Post as Slideshow - One long page vs many short pages?
We are working on implementing a slideshow format for some of the posts on a website, and it appears that using this format breaks a long post into several shorter pages. That's what we want from a user experience standpoint, but are wondering if there are negative SEO implications from having the content broken up in this way, and whether search engines will view it as one longer page or several very short pages? On-Page Report Card: Whats up with the TITLE of the page?
Started to fix the SEO issues on a customers website. When I run a "On-Page Report Card" It says that the title of the webpage:

www.visbymaklarna.se/visbymaklarna.html Is "visbymäklarna - Ditt förstahandsval på gotland." But if I check in the source code of the webbrowser the title should be:
name="title" content="Vi är mäklarna på Gotland som sätter människan i första rummet" /> (Actually this is with special encoding for the swedish characters. The title in coded text is: "Vi är mäklarna på Gotland som sätter människan i första rummet") Anyway the title of the webpage source code and the title of what SEOmoz reports is different. Why does my on-page report card say my page title is 403 forbidden when its not?
I'm trying to get on top of my on page stuff and I'm going through the SEO Moz on-page report cards and it says I'm scoring a fail on certain elements within the 'critical' and 'high importance' factors as my page title is '403 forbidden' but when I go on to my site, my sites CMS it's not '403 forbidden' it's the text I entered?

On-page: Over optimized images?
Hello guys. I have a small question about an on-page optimization for images. What I have: good title tag / good url structure good content (NOT keyword shuffled, its real content, for real people) images / gallery uploaded to folder named same as article name. For example: Great tips for bloggers [article name], great-tips-for-bloggers [folder name]. So my question is: Will Google harm me for this "too good" paths to images, article related image filenames, with mask like [gtips-img01], and if all images have titles / alt tags? Thank you guys.
